PHOTO RELEASE AND AUTHORIZATION:

Confetti's Create and Celebrate is granted permission to use group or individual photo images taken during class/events for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, marketing on social media and web content.  Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

 ABILITY TO ENGAGE IN ART ACTIVITIES AND ASSUMPTION OF THE RISK: 

Confetti's Create & Celebrate takes all possible precautions to reduce risk and provide safe, healthy and enjoyable experiences. I warrant that myself and/or my child is able to follow directions for all activities in the studio/class. I acknowledge that risks from participation in class activities exist and that I have allowed myself and/or child to attend art studio workshops and events knowing these risks and their possible consequences including personal injury. 

  WAIVER AND RELEASE OF LIABILITY:

 I myself as a participant, or as a parent or guardian of my child, agree that I will not hold Confetti's Create & Celebrate  liable for any personal injury, property damage or loss of insurance. I agree to release and hold harmless Confetti's Create & Celebrate and owner, KMO Creative LLC, from all liability incurred as a result of my participation or my  child’s participation in studio class/events and that these terms serve as a release for me, volunteers, property owners and members of my family. If I am filling out this form for a minor, I attest that I am the parent/guardian of the child that I am registering for classes or permitting to participate in classes or events at Confetti's Create & Celebrate.

  WAIVER AND RELEASE OF LIABILITY - Adults 21+:

I understand that I am attending a "craft & sip". I am of legal drinking age (21). I may bring my own unopened container of wine or beer to the studio. I acknowledge that Confetti's is an art studio that allows its participants to BYOB while they attend workshops and does not supply or sell beer or wine to its customers. I will consume alcohol that I bring responsibly, and have been encouraged to take a taxi or carpool if I will be drinking alcohol, and I assume all risk for my actions while under the influence of alcohol. I understand that if there are any concerns about my ability to operate a motor vehicle upon leaving Confetti's, any agent of the studio may call the local police department to conduct a wellness check on my behalf, but does not have an obligation to do so. I understand that I cannot remove an open container of alcohol from the premises and must leave opened bottles behind for disposal.

 MEDICAL EMERGENCIES: 

The undersigned gives permission to Confetti's Create & Celebrate,  its owner and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. If necessary, I request that my child be transported to a nearby hospital.   

I have carefully read this agreement and fully understand all content. I am aware the agreement includes a waiver of liability and indemnity agreement.
